Id go with setting up the instruments first. give it a similar pattern as the original song its from, then mix it up a bit, maybe make it longer, mark where you plan to have the vocals, then splice them up and mix them in.

im still trying to figure that out. lol. I guess it depends on the acapella, the original track, and what sort of music your trying to do.

I like to diagram the parts of the vocal (hook, verse, breakdown, ect.) and then throw in the chord structure. From there I build up the hook.
at least ta skeleton of the drums and main melody first, then I sync the vocal, and build the rest around to vocal for breakdowns or fills or more inst
